What are Special Needs Plans?
Special Needs Plans (SNPs) are a type of Medicare Advantage Plan that provide additional coordinated, specialized care for the most vulnerable Medicare beneficiaries. There are three different types of SNPs:
- C-SNPs – for those with chronic conditions such as dementia, diabetes or stroke
- D-SNPs – for those who are eligible for both Medicare and Medicaid (dual eligible) and generally live in the community.
- I-SNPs – for those who are in institutions such as nursing homes, or who are eligible for institutional care
TIHP: A Special Needs Plan
Texas Independence Health Plan (TIHP) is proud to offer an Institutional Special Needs Plan (I-SNP) that meets the special needs of residents of long-term care facilities in selected Texas counties.
TIHP provides services in the following types of facilities:
- Long-Term Care Facilities
- Skilled nursing facilities (SNFs)
- Nursing facilities (NFs)
To qualify for TIHP, residents should be in one of these facilities for at least 90 days, or be expected to need at least 90 days of institutional care.
